The Care Plan ensures that we can deliver high quality, personcentred care. Once the assessment is completed, we work with residents, families and where appropriate other agencies, to develop a care plan that enables us to provide services that meet your identified needs, using your preferred approach. It will detail the support that you need and the help or services you will get from us. Care plans are reviewed with all parties monthly or as and when changes occur. Personalised care planning empowers individuals, promotes independence and encourages them to be involved in decisions about their care. It centres on listening to individuals, finding out what matters to them and understanding what support they need.

Respite, or shortterm care, offers an opportunity
to sample life at Hazelwell Lodge slowly introducing
the peace of mind and security we provide without
any commitment to a longterm arrangement.

Our flexible respite care arrangements often prove
to be invaluable for older people and their families.
For individuals managing at home alone, a short stay
provides a break from the stresses of coping alone.
For those who require more support respite care
can provide fulltime carers with a welldeserved
holiday, safe in the knowledge that their loved
one is happy and in safe hands.

SPEND SOME TIME WITH US

We offer day care facilities to support those
who may require it, assisting them in remaining
independent and active for as long as possible.
Our day care service can help bridge the gap
between an independent life at home and a move into permanent residential care.

We welcome Day care residents at Hazelwell and find
that, in addition to the benefits of companionship,
genuine friendships are formed between our
permanent residents and those enjoying day
care. We provide day care residents with the
same wide range of activities.

Hazelwell Lodge aims to provide an individual’s final days with comfort, dignity and peace. Supported by the District Nurse and Palliative Care Team, the home ensures effective symptom control and meets individuals’ physical, emotional and spiritual needs while considering their preferences and wishes. The home arranges private, comfortable visits for friends and relatives.

Having dementia doesn’t mean you have to stop doing the things you enjoy. Engaging in physical, mental, social, and creative activities can help assist people to live well with dementia maintain wellbeing.

We believe our individualised programme of activities enhances a person’s sense of dignity and self-esteem. Activities give our residents a sense of purpose, make them feel valued, and enhance their lives.

By tailoring activities to each resident’s personal interests and abilities, we aim to create meaningful experiences that promote a sense of purpose and self-worth.

A variety of live performers where our residents can watch, listen to and take part with our performers and sing songs that they may be familiar with from the past.

We have an intergenerational project (Honeybees) where children visit our residents to socialise and engage in activities together. This aims to improve the quality of life for our residents whilst also providing benefits for the children. These activities offer our residents opportunities for socialising, mental and physical stimulation, emotional support and personal development, and help in establishing relationships between our residents and the local community across generations.

Pet therapy

Pet therapy helps our residents cope with physical health problems and anxiety. The presence of therapy animals reduces stress and increases happiness. It can encourage communication and socialisation skills and provide comfort during stressful times. Depression and anxiety levels often decrease in residents who receive pet therapy.
